And you can do that with 3 points of contact on the ladder?

Three points of contact is for when climbing a ladder, not while working at the top of it.

 

I've always believed it to be when working from it, except where an additional fall arrest system is used.

When working from a ladder where you cannot maintain a handhold other than for a brief period of time (eg to hold a nail whilst starting to knock it in, starting a screw), other measures will be needed to prevent a fall or reduce the consequences of one (eg a fall arrest system)

 

Hmmm...Because DIYers and indeed contractors will always have a fall arrest situation in place.
